New Patients: Setting up Your First Appointment
To schedule your first appointment please e-mail office@seattleacupuncture.com or text (206) 383-9921. When you inquire, please indicate the primary reason you are seeking treatment.
If you have insurance, download the insurance verification form below and confirm your coverage with your insurance company. We will also need your insurance card and date of birth prior to scheduling your appointment.

We have a 48-hour cancellation/no-show policy. You will be charged for your appointment if you cancel with less than 48 hours notice or no-show. A credit card on file is required to secure your appointment.
Location: 2120 1st Ave N, Seattle WA 98109 at the top of Queen Anne at the intersection of First Avenue North and Boston Street. It's a 3 story brick building behind a huge constrction project. Parking: Two free hours of free parking one block South of the office under Trader Joe's
Receipts will be provided to cash paying patients for taxes, HSA accounts and Flex spending accounts at the time of service: not retroactively. If you did not pay with cash, your credit card statement, bank statements, EOB from the insurance company or ledger from venmo or zelle are all acceptable forms of payment when you do your taxes. Superbills will not be provided to patients with insurance plans that we are not contracted with.
